Egenskaper

Se L23 Tabell 22 Flerkammerposer og storposer. Fabrikkfremstilte flerkammerposer og storposer inneholder varierende mengder aminosyrer, karbohydrat, fett og elektrolytter, men ikke sporelementer og vitaminer. Noen poser inneholder fett, andre kan tilsettes fett umiddelbart før bruk. Fett- og aminosyrekilder varierer for de ulike produktene, se produsentenes produktinformasjon for den enkelte sammensetning. Flerkammerposer må tilsettes vitaminer og sporelementer, og vanligvis suppleres med elektrolytter og ev. fett for å bli komplette. Kamrene i flerkammerposene må slås sammen før tilsetninger gjøres.

Tabell 22 viser sammensetningen per liter i fabrikkfremstilte produkter som tilbys. Sammensetningen varierer etter hvilket behov de er ment å dekke, f.eks. som supplement til enteral ernæring eller med en sammensetning som egner seg for infusjon perifert. Mange steder er det også mulig å få apotekfremstilt parenteral ernæring etter individuelle behov. Normalbehov hos voksne per kg kroppsvekt fremgår av Tabell 6 T23 Tabell 6 Basalt døgnbehov (voksne), hos barn av Tabell 7 i T23 T23 Tabell 7 Normalbehov for glukose, protein, fett, energi, elektrolytter, sporelementer og vitaminer per kg per døgn (barn).

Produsentenes grenser for tilsetninger til de ulike posene må overholdes, da det er risiko for utfellinger i vannfasen og destabilisering av fettemulsjonen. Norsk legemiddelstandard anbefaler av hygieniske grunner at tilsetninger til oppløsninger for parenteral ernæring gjøres i apotek. Etter tilsetning i LAF-benk under kontrollerte betingelser på apotek er posene holdbare 4–6 døgn i kjøleskap før infusjonsdøgnet. Ekstra tilsetninger kan gjøres på avdelingen innenfor de angitte grenser for tilsetninger, og posen skal da infunderes innen 24 timer. FDA anbefaler bruk av filter under infusjon for å hindre at ev. utfellinger blir infundert. Det skal brukes 0,2 μm filter til blandinger uten fett, og filter med porestørrelse 1,2 μm til blandinger med fett.
